Influence of hand cycling on physical capacity in the rehabilitation of persons with a spinal cord injury: a longitudinal cohort study
OBJECTIVE: To investigate the influence of http://www.easymedonline.com/Mobility-Scooters-s/20.htm on outcome measures of physical capacity during and after rehabilitation in persons with paraplegia and tetraplegia in The Netherlands. DESIGN: A longitudinal cohort study with measurement moments at the start (t1) and end (t2) of clinical rehabilitation and 1 year after discharge (t3). Hand cycle use was assessed by means of questionnaires at t2 and t3. SETTING: Eight rehabilitation centers in The Netherlands. PARTICIPANTS: Subjects (N=162) with a recent spinal cord injury. INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

Medicare program; conditions for payment of power mobility devices, including power wheelchairs and power-operated vehicles. Final rule
This final rule conforms our regulations to section 302(a)(2)(E)(iv) of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003. This rule defines the term power mobility devices (PMDs) as power wheelchairs and medical scooters power operated vehicles (POVs or scooters). It sets forth revised conditions for Medicare payment of PMDs and defines who may prescribe PMDs. This rule also requires a face-to-face examination of the beneficiary by the physician or treating practitioner, a written prescription, and receipt of pertinent parts of the medical record by the supplier within 45 days after the face-to-face examination that the durable medical equipment suppliers maintain in their records and make available to CMS or its agents upon request. Finally, this rule discusses CMS’ policy on documentation that may be requested by CMS or its agents to support a Medicare claim for payment, as well as the elimination of the Certificate of Medical Necessity (CMN) for PMDs.

BACKGROUND: To evaluate the effect of a 7-week low-intensity hand rim http://www.easymedonline.com/Mobility-Scooters-s/20.htm training on the submaximal metabolic cost, mechanical efficiency and propulsion technique in able-bodied participants. METHODS: Participants were randomly divided over an experimental group (n=14) and a control group (n=7). The experimental group received 7 weeks wheelchair training (3 week(-1), 70 min) at a low intensity (30% of the heart rate reserve), whereas the control group did not receive training. During pre- and post-tests, submaximal exercise was performed on a stationary wheelchair ergometer at fixed levels of power output. Mechanical efficiency, oxygen uptake, heart rate, timing parameters and stroke angles were measured. Video recordings were made to determine the stroke pattern. FINDINGS: Mechanical efficiency increased and metabolic cost decreased significantly in the experimental group compared to the control group. Push time increased and cycle frequency decreased as a result of training. The stroke angle increased in the experimental group during the training period. The experimental group preferred double-looping over propulsion, while the control group mainly used single-looping over propulsion patterns during the post-test. INTERPRETATION: A low-intensity, 7-week training protocol has a beneficial effect on the mechanical efficiency and metabolic cost of medical scooters propulsion in able-bodied participants. OBJECTIVE: To determine the relationship between the resultant force at the pushrim and the net shoulder joint moments during manual wheelchair propulsion in elderly persons. DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and http://www.easymedonline.com/Mobility-Scooters-s/20.htm significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

Evaluation of pushrim-activated power-assisted wheelchairs using ANSI/RESNA standards
OBJECTIVE: To determine and compare performance of pushrim-activated power-assisted wheelchairs (PAPAW) (iGLIDE, e-motion, Xtender) on national standards. DESIGN: Engineering performance and safety evaluation. SETTING: A Veteran Affairs and university-based research center. SPECIMENS: Nine PAPAWs. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Static, dynamic stability, brake effectiveness, maximum speed, acceleration, retardation, energy consumption, static, impact, and fatigue strength. RESULTS: There was no significant difference among the 3 models in forward stability. The iGLIDE was the most stable, whereas the e-motion was the least stable model in the rearward stability tests. All PAPAWs performed equally on the slopes of 3 degrees and 6 degrees in the forward and rearward directions. Braking distance was the highest for e-motion (5.64+/-0.28m) and the lowest (1.13+/-0.03m) for the iGLIDE in forward direction. The average equivalent cycles of all PAPAWs were 318,292+/-112,776.6 cycles (n=8) on the fatigue tests. All PAPAWs passed the impact and static strength tests. CONCLUSIONS: The standards of the American National Standards Institute and http://www.easymedonline.com/Mobility-Scooters-s/20.htm, power wheelchairs the Rehabilitation Engineering and Assistive Technology Society of North America could act as quality assurance tool for wheelchairs. Comparative evaluation of chargers for wheelchair gel cell batteries
This study was undertaken as part of our ongoing work to upgrade wheelchair quality. Laboratory tests were conducted to determine which battery chargers would ensure that the user’s batteries are fully charged each night and also ensure that the battery lifetime was not diminished. Five commercial chargers were tested on a standardised pair of gel cell batteries of a type commonly used to power wheelchairs and medical scooters. Results of this study indicate a clear preference between chargers. As a result, we have changed the chargers we purchase. We anticipate that our clients will now experience longer life from their wheelchair gel cell batteries.
